Questionnaire 6 Child, Ages and Stages (Q6ASQ)
This questionnaire is the third Ages and Stages Questionnaire given at 36 months, and was designed to follow up with participants on previously collected data examining determinants of maternal, infant, and child outcomes. Participants must have completed previous AOB questionnaires (Q1M, Q2M, Q3M, Q4ASQ, Q4CDI, Q5CDI, Q5M, Q5ASQ, Q6CDI and/or Q6M), have a study child of approximately 36 months of age, and not be discontinued from the study

Questionnaire 1 Maternal (Q1M)
This questionnaire is the first maternal questionnaire given at <25 weeks gestation, and was designed to examine determinants of maternal, infant, and child outcomes and identify barriers and facilitators in health care utilization. Participants must be less than 24 weeks and 6 days gestation age at time of recruitment, at least 18 years of age, receiving prenatal care in Calgary, and able to complete the questionnaires in English.

This questionnaire is the third maternal questionnaire given at 4 months postpartum, and was designed to examine determinants of maternal, infant, and child outcomes and identify barriers and facilitators in health care utilization. Participants must have completed previous AOB questionnaire (Q1M or Q2M) and be ~4 months post-partum for study pregnancy, and not be discontinued from the study.
